If you do not bring your identification, your identity cannot be verified, and issuance will not be possible to protect patient information.
Diagnostic certificates can only be requested by the patient in person. Issued documents cannot be refunded or cancelled.
When receiving by proxy:Required documents: family relationship certificate, power of attorney, ID copies (patient and proxy)
Even direct family members are regulated as third parties and must bring a power of attorney. Health insurance cards are not accepted as family relationship documents.We apologize for any inconvenience in issuing certificates.
